Booking before hospital discharge
The single highest-impact thing a family can do for a smoother recovery is to arrange the first home visit before the patient leaves hospital. Discharge day is high-risk: medications change, wound care instructions are new, and the home is suddenly the clinical environment. Having a CareGiver scheduled for day 0 or day 1 dramatically reduces avoidable readmissions.
